Prairie River utilizes a dedicated RN Case Manager to oversee and manage clients suffering with a chronic illness – such as diabetes, heart failure, COPD, asthma and other chronic illnesses – to better manage their health and achieve the best quality of life possible.
Our clinicians and caregivers are specially trained and certified in chronic illness management utilizing a patient-centered approach to help patients in their homes learn ways to manage their illness. Through coaching and education, patients and families learn self-management skills that help them feel more confident to live a healthier lifestyle.
